Self Managed Patient

Patients can be assessed as self managed by their Medical Case Managers. These patients do not have critical needs and can address issues themselves with minimal help. If a Self Managed patient has a concern they can call the resource line which is checked 3 x a day, Monday through Friday.

Responsibilities of an AID Gwinnett/Ric Crawford Clinic Patient

Provide verification of HIV status – antibody test only one time

Provide Verification of Income every six months

Provide Proof of residency every six months

Call to cancel appointments 24 hrs in advance

Dismissal from program after 3 medical appointment “no show – no calls.”
