The objective of case management is to increase the quality of life of individuals living with HIV/AIDS by encouraging self empowerment. This objective is reached by reducing barriers to medical care and social services.
Services include access to healthcare, primary care in the HIV clinic, care plan and assessment, assistance with entitlements, financial assistance, housing assistance, mental health counseling, Buddy Program, AGI Calling Program, nutrition counseling and assistance, transportation assistance.
