We are asking all of the HIV testing sites in the Proyecto Saber program to please help AIDGwinnett by completing our provider survey and then faxing the results to us at 678-990-6429. FREQUENTLY ASKED QUESTIONS
- I need more supplies like purpble bubble sheets, HIV testing kits, forms and other materials. How do I get them?
- Please contact Greg or Kyle. We will arrange to send the materials you need.
- How should I submit my invoices?
- Please submit an invoice no later than the 25th day of each month, for all the tests conducted during that month. Remember that you are required to include all of the completed necessary paperwork: consent forms, purple bubble sheets, lab forms (if using OraSure to confirm a reactive OraQuick result). All forms must be correctly completed; otherwise you may not receive payment for the test conducted.
- I have OraQuick testing kits due to expire "October 2006" but when is that exactly? And can I use kits if they are only 1 or 2 days past the expiration date?
- If the outside of the foil wrapper of the kit says "Expires October 2006" this means October 31. You cannot use an expired kit - even if only 1 or 2 days expired. Please contact us to arrange for return to AIDGwinnett. Also, please do not allow kits to expire. If you see you have kits that are due to expire within 3 weeks, and you don't think you will be able to use those kits before the expiration, please call us to arrange a return because we can use them quickly at other locations and thus avoid wasted supplies.
